If this is the second passport for a cadet, the school PMS will need to download the ESTP Rollover/Additional Passport Request form below and e-mail or fax your DEC the following information. If a waiver is requested, follow the directions under 1.b on the form.
Whether you are requesting a waiver or not, the form must be filled out BEFORE a Cadet can get a new passport. If not requesting a waiver, only send the form to your DEC.
